Hazard: The recalled Halloween figurines contain high levels of lead in the
paint.
Description: The recalled Casper the Friendly Ghost® mini-figurines are made of
resin and paper, and are three inches tall. They were sold in mini bags
decorated with a spider and web design. The Casper the Friendly Ghost® figurines
are dressed as: Dracula in a red spider bag, Skeleton in a green spider bag,
Devil in a purple spider bag, Mummy in an orange spider bag, Casper® in a blue
spider bag, and Wendy the Witch® is in a yellow spider bag. The figurines are
stamped with TM & ©Harvey Ent. “Coyne’s & Company” and “CP1030” are printed on a
white sticker on the bottom of the bags.

Cybex International Recalls Treadmills Due to Fall Hazard; Can Speed Up
Unexpectedly
Hazard: The treadmills can speed up unexpectedly while in use due to a
malfunction with the lower control board, posing a fall hazard to consumers.
Description: The recall involves the Cybex 445T, 455T, 530T, 450T, 500T, 515T,
and 520T treadmill models. The treadmills are black and gray with rectangular
uprights. The 530T style treadmill is 81 inches long by 32 inches wide. The 445T
style treadmill is 72 inches long by 32 inches wide. The treadmills have a
display panel on a console as wide as the treadmill. “Cybex” and the model
number are written on the console.

Serta International Recalls Mattresses Due to Violation of Federal Mattress
Flammability Standard
Hazard: The mattresses fail to meet the mandatory federal open flame
standard for mattresses, posing a fire hazard to consumers.
Description: The recalled mattresses have a zipper cover over a foam core. The
mattresses were sold in twin, full, queen, king and California king sizes. They
were manufactured between November 30, 2007 and May 20, 2008. The mattress name,

Hazard: The multi-purpose lighters fail to meet federal safety standards because
they might not contain a child-resistant mechanism, posing a potential fire and

Description: The recalled “Gas Match” lighters have either a multi-colored
red/white, blue/white or green/white plastic body, and a silver-colored metal

Burn Hazard
Hazard: The rechargeable batteries included with the Recharging Station can
overheat if the gaming system is placed into the recharging base upside down,

Description: The Recharging Station comes with two rechargeable batteries, an AC

Hazard: Rim surface and spoke hole plugs on the wheel can cause a puncture to
the inner tube, resulting in a flat tire. This can cause the rider to lose

Description: This recall involves the Shimano Dura Ace Carbon Clincher Wheel
Sets with model number WH-7850 C24CL sold for road racing bicycles. The rim has

Hazard: A circuit board defect can cause AA batteries used in the guitar to leak

My Way Corp. Recalls Baby Walkers Due to Stairway Fall Hazard
Hazard: The walkers violate the baby walker voluntary standard and can fit
through a standard doorway and are not designed to stop at the edge of a step.
Babies using these walkers can be seriously injured or killed.
Description: The recalled walkers were sold for babies 6 months and older. The
walkers were sold in the following colors: pink, red, green, blue, and ivory.

Fujifilm Recalls Battery Chargers Due to Shock Hazard
Hazard: The plastic portion of the unit’s power plug can crack or detach, posing
an electrical shock hazard to consumers.
Description: The recalled BC-50 battery chargers were included with the Fujifilm
FinePix F100fd digital cameras and are identified by the production lot codes
“WCAA” and “WCAB”, which can be located on the back of the battery charger unit,
in the lower, right-hand corner. The battery chargers were also sold separately
as an optional accessory for the FinePix F100fd and F50fd digital cameras and

Toy Boats Recalled Due to Burn Hazard
Hazard: The two battery terminals can come into contact with each other, causing

Description: The recalled toy speed boats are lightweight plastic toy boats

Hazard: The automatic gas vent damper could fail, and if the blocked vent switch
does not activate, the vent could leak carbon monoxide (CO). This poses a risk
of CO poisoning to consumers.
Description: The recall involves Effikal RVGP-PC Gas Vent Damper size 4, 5, 6,
7, 8, 9, or 10 which are installed with the gas boiler systems listed below. The
vent damper date code is located on a sticker under the motor assembly cap on
the plastic base. Serial numbers are located on a label on the side of the

Hasbro Inc. Recalls to Repair Nerf Blasters; Child’s Skin Can Get Caught in
Plunger of the Toy
Hazard: The blaster’s plunger can pull the user’s skin during firing of the toy
blaster resulting in injury to the face, neck, and/or chest.
Description: This recall involves the Nerf N-Strike Recon CS-6 Blasters for
children age 6 and up. The toy blaster is yellow with a black handle and orange
plunger, trigger, and reload clip. The word “NERF” in black lettering is on both
sides of the blaster and the word “ARMED” is indented on the orange plunger.
“RECON CS-6” is on the gray cocking mechanism. There are five interchangeable
parts including the shoulder stock, the flip-up sight, barrel extension, quick

Hazard: Due to sizing problems with the crib’s hardware, the drop side can come
off the tracks. When the drop side detaches or partially detaches, it creates a
hazardous gap, which can lead to infant entrapment and suffocation.
Description: This recall of Simplicity drop side cribs involves models that used
a different style of hardware from those cribs recalled in September 2007. The
recalled model numbers include: 8620, 8745, 8748, 8755, 8756, 8778, 8810, and
8994. The recalled drop side cribs have a date code, which can be found on a

Regent Sports Recalls Soccer Goal Nets Following Strangulation Death of a
Child
Hazard: The fixed knot flexible openings in the soccer goal net can pose a head
and neck entrapment or strangulation hazard to young children.
Description: Both soccer goals have a foldable white frame with a white net that
is attached by Velcro strips. When upright, the MacGregor goal measures 6 feet

PB Heat Recalls Gas Boilers Due to Carbon Monoxide Hazard
Hazard: The pressure switch could have an internal defect that could fail to
shut down the boilers under vent or air inlet blockage. This switch defect could
allow the boilers to produce excessive levels of carbon monoxide, posing a risk
of carbon monoxide poisoning to consumers.
Description: This recall involves the DE and PSC II Gas Fired Cast Iron Boilers
with the Endura Plastics Model ES2035 pressure switch used for home heating and
domestic hot water heating. The boilers were available in either natural or LP
gas. The boiler rating label is located on the top jacket panel on Series PSCII
models and the left side jacket panel on the Series DE models. The label

Aqua Lung Recalls Scuba Regulators and Adapters Due to Drowning Hazard
Hazard: Over-tightening of the DIN retainer by a technician during installation
can result in the retainer breaking under pressure, a rapid escape of air from
the scuba cylinder, and the regulator detaching from the scuba cylinder. This
poses a drowning hazard to divers.
Description: The recalled regulators have a brass DIN retainer manufactured
prior to June 2006. The recalled Aqua Lung Titan Din 1st Stage regulators have
serial numbers lower than 6062501 stamped on the side of the regulator’s body.
Recalled Titan/Conshelf DIN adaptors are marked “300 BAR MAX” on the side of the

Hazard: Irregularly positioned wires near the computer’s hinge and/or a

Hazard: The splice can fail to hold the wires adequately together, posing a

Description: The recalled butt splices are used to connect electrical wires to
one another. They are typically used for wiring small electrical appliances,
like audio equipment, or in automotive applications. The splices are yellow
insulated vinyl and measure about one inch long and ¼ inch wide. They were

Electra Bicycle Company Expands Recall of Amsterdam Bicycles; Chain Can
Derail and Pose Injury Risk to Riders
Hazard: The interior alignment tabs of the bicycle’s chainguard can be pushed
against the chain causing it to derail, which poses a risk of injury to riders.
Description: This recall involves some Amsterdam model bicycles in the Classic
3, Original 3, Royal 8, and Sport 3 styles. The bicycles have an enclosed
chainguard. “Electra” and “Amsterdam” are printed on the bicycle’s frame and/or

Devices for bicycles and includes models Hollow CroMo Front 100mm, Hollow CroMo
Rear 130mm, and Hollow CroMo Rear 135mm. These devices have hollow skewers. They
were sold individually or with SunRinglé front and rear wheels with model names
Black Flag, Disc o Flea, Charger, STR8 Track, Accelerator R3.00C and Accelerator

Counterfeit Circuit Breakers Recalled By Specialty Lamp International Due to
Fire Hazard
Hazard: The recalled circuit breakers labeled “Square D” have been determined by
Square D to be counterfeit and can fail to trip when they are overloaded, posing
a fire hazard to consumers.
Description: The counterfeit circuit breakers are black and are labeled as
Square D QO-series models 115, 120, 130, 215, 220, 230, 240, 250, 260 and 2020
and Square D QOB-series models 115, 120, 130, 220, 230, 250, 260 and 1515.
Actual Square D circuit breakers have (a) the amp rating written on the handle
in white paint on the front of the breaker (authentic Square D circuit breakers
manufactured prior to 2003 did not have white paint on the amperage numbers);
(b) the Square D insignia molded onto the breaker side, and; (c) a yellow
chromate mounting clip with half of the top of the clip visible. If your
breaker, labeled as Square D, does not match this description, it could be
counterfeit.

Alarms Due to Alarm Malfunction
Hazard: The recalled alarms can sound a “double chirp”/fault alarm in the
presence of CO, prior to going into full alarm. Upon hearing a double chirp, the
Owner’s Manual instructions recommend a consumer take the alarm out of service,
which could expose consumers to hazardous levels of CO and suffer injury or
death.
Description: The recall includes FireX branded 10000 and 12000 series alarms

Hazard: When switched OFF, one electrical pole may remain energized, posing a

Description: The recalled toggle and rotary switches are manual motor controller
switches or disconnect switches. The switches are typically used with HVAC
